We'll begin this tutorial, by creating a new canvas at a size of 281 x 129 then fill with 6aa3cf as the color. Double click the background layer in the layers palette and click OK to unlock.

Next, download this pattern.
  Now right click on the background layer and select, "Blending Options...". Now click on "Gradient Overlay".

Do not click "OK" yet. Still more to do.
  Now click on "Pattern Overlay".

You may now click OK.
You should now have the below image.
Next, select the "Rounded Rectangle Tool". Draw a rectangle as illustrated below with a radius of 5 pixels and color 2a425f. Name this layer "Button".

  Now right click on the "Button" layer and select, "Blending Options...". Now click on "Drop Shadow".

Do not click "OK" yet. Still more to do.
  Now click on "Inner Glow".

  Now click on " Color Overlay".

  Now click on "Gradient Overlay".

  Now click on "Stroke".

You may now click OK.
You should now have the below image.
Next, select the "Custom Shape Tool" and select the "Envelope 1" shape found in the drop down in the option palette as illustrated below.

Make sure the Foreground color is a1c9f7 and draw your shape. Name this layer "Envelope".
  Now right click on the "Envelope" layer and select, "Blending Options...". Now click on "Outer Glow".

Do not click "OK" yet. Still more to do.
  Now click on "Inner Glow".
  use settings at left
You may now click OK.
You should now have the below image.
Select the "Text Tool" and use the below settings:
Now type "Send us an email Today!" or the text that you prefer.
  Now right click on the text layer and select, "Blending Options...". Now click on "Drop Shadow".

You may now click OK.
You should now have the below image.
Now select the "Pen Tool" and draw a curve effect as illustrated below using White as the color. Name this layer "Gloss".
Now "Ctrl" click the "Button" layer to create a selection around it and go to "Selection" then "Inverse". Now rasterize the "Gloss" layer that you drew with the "Pen Tool", then go to "Edit" and lastly "Cut".
The Results!!!
Turn the Fill of the "Gloss" layer down to about 40% and your finished.
