We'll begin this tutorial by going to the main menu and selecting "File" then "New..." and typing 64 for both the width and the heigth. Click "OK". Next, select the "Rounded Rectangle Tool" and draw a shape with a radius of 100 pixels like shown below.

Next, add some anchor points using the "Add Anchor Point Tool" as I have done below. Click the middle anchor point on the top and bottom of the shape and bring up/down five pixels using the up/down arrow keys on your keyboard. |

Now, go to the main menu above and select "Edit" then "Free Transform". While holding in the "Shift" key on your keyboard, angle the shape as I have illustrated below by dragging your mouse either up or down along any outside corner of the transformation selection. |

Now right click on the shape layer and select, "Blending Options...". Now click on "Drop Shadow". |

Now click on "Inner Shadow". |

Now click on "Gradient Overlay". |

After completing fig. 1 you may click OK. |

You should now have the following: |

Now, duplicate the layer, by going to the above menu and selecting "Layer" then "Duplicate", and go to the main menu again and select "Edit" then "Transform" and lastly "Horizontal" and move to the appropriate place. Right click the new layer in the layers pallete, select "Blending Options" and select "Inner Shadow". Change the "Angle" to 141. Now select "Gradient Overlay" and change its "Angle" to -135 and click "OK". |

Now, go to the main menu again and select "Edit" then "Define Pattern" give it a name and click "OK". Create a new document at any size and fill with the new pattern by going to the main menu and selecting "Edit" then "Fill". Change the "Use" option to pattern, select the newly created pattern and click "OK". |

Lastly, go to the main menu one final time and select "Filter" then "Render" and then "Lighting Effects" and use the below settings and click "OK". Go to the main menu again and select "Edit" then "Fade Lighting Effects" and input 50 for percentage and click "OK". |
