We'll begin this tutorial, by selecting the "Rounded Rectangle Tool". Draw a rectangle as illustrated below with a radius of 5 pixels. Color doesn't matter.
  Now that you have your shape, you can now right click on the layer and select, "Blending Options...". Now click on "Drop Shadow".

Do not click "OK" yet. Still more to do.
  Now click on "Inner Shadow".

  Now click on "Gradient Overlay".
  use settings at left and in Fig. 1.
You may click "OK" after step 5.

You should now have the below image.
 
Select the "Text Tool" and use Verdana typeface at a size of 10, Bold and all caps as illustrated below using 5fbfef as the color. Set the Tracking to 40.
  Now that you have your text, you can now right click on the text layer and select, "Blending Options...". Now click on "Drop Shadow".

You may now click OK.
Duplicate the text layer and place accordingly then you should have the below image.
Now grab the "Line Tool" and set the weight to 1 pixel. Draw the line now while holding the "Shift" key in on your keyboard to make a straight line using 55bbee as your color.
  Now that you have your line, you can now right click on the line layer and select, "Blending Options...". Now click on "Drop Shadow".

You may now click OK.

You should now have the below image.
Using the "Pen Tool" draw the below shape with white as your color.
Now "Ctrl" click the menu layer to create a selection around it. Then, go to the main menu and click on "Select" then "Modify" and then "Contract". Contract by 2 and click "OK". Then go to the main menu once again and click on "Select" and lastly "Inverse". Now rasterize the new shape layer that you drew with the "Pen Tool", then go to "Edit" and lastly "Cut".
The Results!!!
Turn the opacity down on the glare layer and your done. I hope you enjoyed this tutorial.
