We'll begin this tutorial, by creating a new canvas at a size of 340 x 155 using White as the color.
  Now right click on the background layer in the Layers Palette and select, "Blending Options...". Now click on "Color Overlay".

Do not click "OK" yet. Still more to do.
  Now click on " Gradient Overlay".

You may now click OK.
You should now have the below image.
Select the "Text Tool" and use the below settings:

Now type your prefered text.
  Now right click on the text layer in the Layers Palette and select, "Blending Options...". Now click on "Inner Shadow".

Do not click "OK" yet. Still more to do.
  Now click on " Bevel and Emboss".

Do not click "OK" yet. Still more to do.
  Now click on " Gradient Overlay".

  Now click on "Stroke".

You may now click OK.
You should now have the below image.
Now, double click the text layer in the Layers Palette that you just created then click the "Create Warped Text" icon in the options bar and use the below settings.

Then, duplicate the text layer and clear its layer style. To do this, right click on the duplicated text layer in the Layers Palette and choose "Clear Layer Style" from the menu.Then drag so it is just above the background layer but below the original text layer in the Layers Palette. Change the duplicated text layers color to 072873.

Next, while on the duplicated text layer in the Layers Palette, hold in the "Alt" key on your keyboard and press the down arrow key once then let go of the "Alt" key and press the left arrow key once. Do this a few times to get the below effect.

  Now right click on the very bottom text layer in the Layers Palette and select, "Blending Options...". Now click on "Drop Shadow".

You may now click OK.
You should now have the below image.
Duplcate the very top (original) text layer and clear its layer style as shown in step 14 and change the color to White. Then rasterize the white text layer.
Next, "Ctrl" click the white text layer in the Layers Palette to create a selection. Now go to "Select" then "Modify" then "Contract" by 1 pixel. Go to the main menu once again and choose "Select" then "Inverse". Lastly, "Edit" then "Cut".
Using the "Pen Tool" draw the below shape with white as your color.

Then "Ctrl" click the new shape layer in the Layers Palette. Be certain that you have the top white text layer selected in the Layers Palette, then go to "Edit" then "Cut". Turn the Fill down to 32% and your done. I hope you enjoyed this tutorial.
